Good Neighbor Community Service Intensive Care Coordination is service for children who are in out-of-home placements or are at risk of such a placement. The service provides individualized goal planning for the family and child as well as an intense level of coordination between family members, professionals, and natural supports to support them in reaching their goal. The focus of service is to have the child safely remain in or return to their home or community.
