Geriatric Care Management

Personal Nurse Consultants offers care management services.  The individual elder care assessment service includes care planning assessments; screening, arranging and monitoring home care; referral and coordination of appropriate health professionals, patient education, chronic disease management, crisis intervention, coordination of community resources.

Staff provides you with advice and guidance for services and resources that are based on what you need.

Transportation is provided to and from medical appointments only if RN is attending the appointment with client.
