Adult Case Management

The long-term community-based Case Management Program encompasses identifying individual and family needs and planning for the current and future. By identifying and coordinating community services to meet the needs and wants of the survivor, monitoring progress, re-evaluating the individual's program, and building support teams to assist the individual and their families, case management participants reach their goals.

The case manager supports the individual as he/she plans for the future and moves toward living as independently as possible in their community. The individual establishes goals based on their wants and needs; the case manager supports them in person-centered planning and through individualized service plans.
