ElderCare Connections offer a care management program.  Geriatric Care Management provides:

• In-Home Assessments and Evaluation of Needs
• Arrangements and Monitoring of Home Care Services and/or Adult Day Care Services
• Coordinate Comprehensive Physical and Mental Evaluations for optimum health and well being
• Monitoring and Coordinating Service Providers
• Evaluation of Housing Alternatives
• Referrals to Attorneys, Physicians, Financial Advisors
• Informal Counseling
• Crisis Intervention
• Coordination of Transportation Services
• Informing and Educating Clients about Medical, Legal and Financial Matters relating to Seniors
• Companionship Services
• Long Distance Care Giving support and frequent updates
• Insurance Counseling: Medicare, Medicaid and Long Term Care Insurance
• Referrals and Coordination of Public Programs
• Long-Term Care Placement, Assessments and Evaluation of Needs
• Help with choosing a long-term care facility, as well as assisting with respite care needs and/or permanent placement and how to ensure your loved one receives quality care. 

Advocates for the clients directly in resolving complaints made regarding long term care facilities, educates consumers and long term care providers about residents' rights and good care practices, provides information to the public on nursing homes and long term care facilities, and assists in the development of citizen organizations, and family councils
