Medical ID and Safety Alert ID Bracelet

Vendor for Durable Do Not Resuscitate (DDNR) bracelets, an EMS program. Appomattox Drug Store is the only State vendor approved by the Commonwealth of Virginia to provide DDNR bracelets.   Durable stainless steel bracelets/necklaces replace the old plastic bracelets. All persons who obtain a DNR order should also apply for the bracelet/necklace to ensure their wishes are met during an emergency.



With the original application for DDNR bracelet or necklace, there must be included a valid DNR approved form from a doctor, a photo ID and one other ID.  Bracelets or necklaces are engraved and sent with the proper identification and copy of DNR order.  Orders cannot be taken over the phone or faxed. Order forms can be mailed, faxed or downloaded from the website for patients to complete.



Everyone with a medical condition or allergy should have a medical id bracelet and/or wallet card.  Call for information about the medical id bracelet.



Supplies can be ordered online, by mail, by telephone and by secure fax.  All orders are shipped within 72 hours by ground service.
