Who We Are
To request a copy of your COVID-19 vaccination record, contact the Virginia Immunization Information System (VIIS) using their online request form. Your vaccine record is available as a scannable QR code to carry on your mobile phone or device. The record is also able to be printed for scanning.
Businesses may download the Smart Health Verifier App from the Apple App Store or Google Play which scans the QR codes to verify an individual's vaccination status.
