LEAF Behavior Support, LLC is a team of Behavior Consultants (PBSF, BCBA, BCaBA, LCSW, LPC, etc.) that provide support to individuals and their families impacted by an intellectual, developmental, mental health diagnosis, or autism to develop person-centered behavior plans to increase or decrease targeted skills.
LEAF is a learning environment for all families. Services are offered Statewide via Telehealth.

Other Eligibility Criteria
Ages 5 and older. Individuals must have an Intellectual and/or Developmental Disability or Mental Health Diagnosis, or access to learning is impacted by behavior challenges.
Intake Process
Families who desire services can coordinate with their case manager/support coordinator or community-based provider to complete a referral. Referral forms can be requested via email or via the website.
