Who We Are
Milestone Group Homes and Nursing offer group homes in Annandale and soon to expand to surrounding areas, for individuals with special needs, autism or intellectual challenges, and physical or developmental disabilities. Their group homes provide congregate living arrangements to a small group of adults with recognized developmental disabilities. Individuals receive person-centered and individualized primary care, which is comprised of a room, board, and general supervision. In addition, they receive assistance training, and specialized supervision according to their individual support needs and preferences. Each individual is encouraged to be as independent as possible with daily living activities hopefully to achieve an improved quality of life through integration in community activities.
Milestone Group Homes and Nursing's professional direct support personnel assist individuals to accomplish these goals:
- use observation, conversation, and other interactions as necessary to develop a functional analysis of the person's capabilities within the first month of residency
- prepare a support plan with measurable outcomes using the functional analysis, the IHP or ISP and plan of care, to develop and maintain as appropriate
- report quarterly to the person, family, guardian, and DBHDS the outcomes of the programming and support provided to help the person to achieve their goals
They ensure that each person receives hands-on support, habilitation, and other supports including:
- eating and drinking
- toileting
- personal hygiene
- dressing and grooming
- monitoring health and assistance with medication
- communications
- interpersonal and social skills
- home management
- mobility
- time and financial management
- academic, motor, and perceptual skills
- human sexuality
- opportunity for social, recreational, and religious activities

Milestone Group Homes & Nursing ensures that each individual receives the professional services required to meet their goals including medicine, dentistry, education, nutrition, and much more. They work closely to be in compliance with DBHDS policies and are always a member of the individual's interdisciplinary team.
