Resources for Children and Youth with Special Health Care Needs, Northern Virginia

Care Connection for Children (CCC) is affiliated with Inova Fairfax Hospital for Children and sponsored by the Virginia Department of Health.  This is a statewide network of regional programs that provides health care coordination, community support and resources to children who have chronic physical health care needs.



The care coordinators offer and provide information and referrals for medical visits, accessing medical services, counseling, health care payment options, school issues, social supports and community resources.



Staff offers advice, education and support to all families regardless of income requirements.  The goal is to help families coordinate community and education resources with medical expertise to ensure that children with special health care needs can reach their maximum potential.
