Geriatric Care Manager offers care planning, resource and referral development, physical and psycho-social assessments, coordination of services and insurance case management. Qualified care managers may help people who have physical disabilities; developmental disabilities; autism; brain injury; mental health problems; or chronic illness.
Bridges is comprised of nurses, social workers, occupational therapists, respiratory therapists, money managers, and a long term care administrator. They provide interdisciplinary care management services to seniors.
Their mission is to provide health care information and options available to promote independence and autonomy. They provide guidance through the changing health care maze and advocate for the rights of all seniors. Also, assist families of loved ones currently living in long term care facilities in ensuring that the needs of their loved ones are being met. Other services offered are home safety evaluations, medical claims filing and appeal, crisis intervention and resource and referral development.
